If you are a new patient to our office, completing the forms prior to your appointment will allow us to attend to your dental needs more quickly than completing them on your arrival.  Please click on the link below (TDO) to get started.  If you are unable to complete the forms online, please arrive 20 minutes prior to your scheduled appointment to allow time to fill out the necessary forms.
